The JRPass is valid for all the JR East group network, excluding the Nozomi bullet train service. All other bullet trains may be used by a JR Pass holder.
About Disneyland, yes, the Pass will take you to Maihama where you can board the Disney private (free) railway from the gates into the park. Here is an sample itinerary for you to use as reference:
