Suffix on Passport

For Last Name/Surname on the Passport field, do you include the suffix if the last name has a suffix or not?

Hi there,

If by suffix you mean something like PhD, please include it ONLY if it is present on your passport. Otherwise, write your full name as it appears on your passport without the suffix.
