Does the JR Pass cover all six regional JR companys?
Can the cost increase when exchanging voucher if exchange rate changes?
Lastly to go to Fuji from Tokyo, the last part not cover by JR pass,
do you buy ticket befor boarding or on train,or at station where you change trains?

Yes the JR Pass covers all regions of Japan, including Hokkaido, Kyushu and even Shikoku.
Can the cost increase when exchanging voucher if exchange rate changes? No, you buy the JR Pass in Yen the moment that you place the order, this means that you won't have to worry about exchange rates. We convert the yen to your local currency when you order.
Lastly for Tokyo - Fuji, you pay for the last part (1,100) yen at Otsuki station before boarding the Fujikyu line heading to Fuji. There's signs everywhere there with ''JR Pass not valid for the Fujikyu line'' because many foreigners try to slip aboard without getting a ticket, you can't miss it!
