Hi, we are spending 21 days in Japan, and I am considering whether or not to buy JR pass for 14 days, and then a PASMO card (is that what they're called?) for use in Tokyo, OR should I buy full 21 days? We may need subway in Tokyo a lot, so if we're not using JR lines that much and would need subway more, what is the cheapest option? Thank you

I should add though that we are planning on travelling to Hakone (for Fuji) and we are also planning on maybe doing a day trip to Nikko.

Local travel around Tokyo is relatively cheap and I think that it would be better to buy normal tickets or day passes to cover your travel if you just stay local. The JR Pass is great if you plan to explore Japan and use it to travel between different cities such as Kyoto and Hiroshima.
There are a lot of different day passes available for Tokyo, so I would look into those. One of the best is the Tokyo Metro Day Pass which is just 710 yen and allows for unlimited travel on the Tokyo Metro for a full day.
You can also buy an IC Cards such as PASMO or Suica to make your travel around the capital more convenient, these are prepaid travel cards which take the need away to buy a ticket every time your travel (they don't give a discount by themselves however).

What if from Tokyo we will still use Jr to get to Nikko? Its still cheaper to buy a return ticket yes? As for Fuji san day trip I think we will buy hakone free pass. Is going to Nikko and Hakone and using the subway still cheaper than the difference between the 14 and 21day pass?

For Nikko:
I would recommend looking into using the Tobu Railway from Tokyo - Nikko. The train connection is only 1320 yen for one way and is a very economic way of traveling to Nikko.
For Hakone:
The Hakone free pass, is a great option and will cover all your travel from Shinjuku into Hakone. This together with the above train to Nikko will still be a better option than going for a JR Upgrade.
