- Narita airport terminal 1 > Tokyo: ¥3,140 (62mins, 79km)
- Tokyo > Nikko: ¥5,630 (121mins, 150km)
- Nikko > Tokyo: ¥5,630 (117mins, 150km)
- Tokyo > Otsuki: ¥2,860 (85mins, 87km)
- Otsuki > Tokyo: ¥2,860 (76mins, 87km)
- Tokyo > Osaka: ¥13,950 (194mins, 556km)
- Osaka > Nara: ¥780 (59mins, 48km)
- Nara > Osaka: ¥780 (55mins, 48km)
- Osaka > Hiroshima: ¥10,150 (104mins, 345km)
- Hiroshima > Osaka: ¥10,820 (109mins, 345km)
- Osaka > Kyoto: ¥540 (29mins, 42km)

As you can see you'll make excellent savings with a 14 day JR Pass, in addition any further JR travel will be free (for example you can make good use of the JR Pass around Tokyo - see this map of JR lines in Tokyo for more)
Please note that I have assumed you'll be visiting Kawaguchiko for Mt. Fuji. For Kawaguchiko the JR Pass will take you as far as Otsuki. From here you'll need to use the non-JR Fujikyu lines up to Kawaguchiko itself. Also, for Koyasan you'll need to use the non-JR Nankai rail from Osaka Namba station.
