Tokyo to Mt Fuji, Toyko to Yokohama, Tokyo to Kyoto (10/15), Kyoto to Hiroshima (10/19), Hiroshima to okayama (10/21), Okayama to Narita (10/23). Toyko-Kyoto-Hiroshima

Your trip looks like a very good base! I guess you'll be looking at a 14 day pass for your trip. Here is an itinerary breakdown to check a 14 day pass will be economical:
- Narita airport terminal 1 > Shinjuku(jr): ¥3310, (84mins, 89km)
- Shinjuku(jr) > Otsuki: ¥2290, (85mins, 77km)
- Otsuki > Tokyo: ¥2860, (76mins, 87km)
- Tokyo > Yokohama: ¥1460, (23mins, 28km)
- Yokohama > Tokyo: ¥450, (27mins, 28km)
- Tokyo > Kyoto: ¥13420, (164mins, 513km)
- Kyoto > Hiroshima: ¥10990, (112mins, 380km)
- Hiroshima > Okayama: ¥5860, (41mins, 161km)
- Okayama > Narita airport terminal 1: ¥18650, (330mins, 812km)
JR ticket cost: ¥59290 for 9 legs
- 14 day rail pass (¥45,100) saving: ¥14190
As you can see you'll make a minimum saving of ¥14,190 with a 14 day pass in addition to retaining flexibility. Because of this I can certainly recommend the 14 day pass option.
Here are some more thoughts on your itinerary:
Mt Fuji (Kawaguchiko)
For Mt Fuji you should head to Kawaguchiko. The JR Pass will take you as far as Otsuki station. From here you'll need to use the non-JR Fujikyu lines up to Kawaguchiko, which wil be around ¥1110 each way.
Recommended extra trips
I recommend the following locations - those that will benefit from an overnight are marked with a *:
- Nikko (While in Tokyo)
- Koyasan (while in Kyoto) *
- Nara (while in Kyoto)
- Miyajima (while in HIroshima) *
- Kinosaki Onsen (while in Kyoto/Okayama) *
- Naoshima art island (while in Okayama) *
- Kurashiki (while in Okayama)
All are accessible with the JR Pass with the exception of Koyasan and Naoshima, which will require extra fees on non-JR parts)
Use of the JR Pass in Tokyo
You can get great use of the JR Pass around Tokyo and visit many of the sight seeing spots. Here's a map of JR lines in Tokyo for reference.
