I am planing a 2 week tour of japan
I week is based in tokyo area
The second week I wish to travel to Hakodate,sapporo and otaru
Maybe spend a day further in onsen country
We are traveling as a family of four

Hi there!
Will you be using the train to travel to Hakodate, Sapporo and Otaru? In that case either a 7 day or 14 day JR Pass would be a great choice. You'd be making great savings just by visiting Sapporo alone.
I'd recommend a 7 day JR Pass if you can make the return Tokyo - Hokkaido in one week, however a 14 day JR Pass would not be bad either.
