Yes, you plan looks do-able. As a suggestion, as Osaka and Kyoto are so close (10-20mins by JR train), you may want to spend both nights in one of them and travel by train as needed. This way you won't need to move your luggage around so much.
Assuming you do your itinerary as is, here is your cost breakdown from the 12-18th.

As you can see you'll make a small base saving with the 7 day JR Pass. I have not included and JR travel around Tokyo - if you plan well you can make great use of the JR lines about tokyo and save around ¥500-1000 per day. You are also free to do as much further JR travel as you need during the validity period.
Please note that the JR Pass will take you as far as Odawara for Hakone, from Hakone you should purchase the Hakone Free Pass to use the non-JR lines that will take you upto and around Hakone. For Disneyland you should use Maihama station and then walk to the entrance from there.
