Hi there!
Yes the full route is covered by the JR Pass, no need for extra tickets!
Just head to Tokyo Station and board the Tohoku Shinkansen there to Aomori - from there you can travel on by limited express train to Sapporo. Do note that the full journey 9.5 hours, so plan accordingly!
