I hold the 21 days Japan Rail Pass. If I plan to travel from Shinjuku to Kawaguchiko by using Ltd. Exp. Fuji Excursion, is that I need to pay extra fare ¥1140 + reserved seat ¥600 for the section Otsuki to Kawaguchiko?

Hi there,
Yes, the route Otsuki to Kawaguchiko is not included, as it is not part of the JR Network.
