Hi there,
I would not recommend this route at all, as it is long and involves multiple transfers.
That said, if you do want to do it, here are your two options:
By Rail:
Take the Shinkansen to Shin-Yokohama, then a local train to Hachioji, followed by a local train to Hachioji, followed by a local train to Otsuki and from there to Fujikyu railway to Kawaguchiko.
Alternatively, you can take a Shinkansen to Mishima station, and from there use an express bus to Kawaguchiko. Neither the bus nor Fujikyu railway are included in the JR Pass and both routes take about 5 hours each. The best route depends on the time you travel and transfer times involved.

This route looks very long indeed.
Would you suggest we do Kyoto -> Nagano -> Mt Fuji -> Hakone -> Tokyo instead?
Tokyo has to be our last leg because we are flying home from NRT airport.
But the sequence of Nagano, Mt Fuji & Hakone can be mixed up.

Sorry for the late reply - I did think I answered and was rereading the post and realized I missed it.
If you still need it, then this proposed route through Nagano would be easier. Though going to Mt. Fuji (I assume you want to go to Kawaguchiko) will take some time. Even better may be to go to Hakone first and lastly to Kawaguchiko.
