That is a lot of travel for one week. While all certainly possible, I feel that it would also be nice if you had some extra time in Tokyo or Kyoto. Of course this way you make great use of the JR Pass.
One tip I could give you if you really want to make most of your JR Pass is make a day trip from Tokyo to Sendai as this city has a lot to offer or make a visit to the close by Matsushima. Both are completely covered by the JR Pass and you get to ride the Tohoku Shinkansen up north! You could do this instead of Hakone, because the JR Pass is not valid inside the Hakone Area itself so you would be able to save some money there too. On the other hand I am not sure if Hakone is to be missed!
And one more tip if you really want to go crazy and all out...Combine Osaka and Nara in a day where you visit Nara in the (early morning) and than move to Osaka to enjoy the evening atmosphere of Shinsaibashi and have a good dinner in the city. Than the next morning you can make a day trip to Hiroshima which certainly is not to be missed either if you visit Japan, you can visit the Peace Museum, Peace Memorial Park and make a visit to Miyajima which is also covered by the JR Pass.
