Could you advise on the best pass for an 11-day round trip starting at Kansai, taking in Hakone, Kyoto, and Amanohashidate.
I have heard that part of the journey to Amanohashidate is not on JR trains so will need a extra fare.
Are any ferries / buses in these areas included?
(I visited Miyajima last year and was delighted that my JR pass covered the ferry.)

Good question and an interesting one as well. Let's have a look at what tickets would cost without the JR Pass and see if one could be of use.
Osaka - Hakone (Odawara station) ¥ 12,270
Odawara - Kyoto ¥ 11,750
Kyoto - Amanohashidate ¥ 4,380 of which ¥ 3,000 is covered
Amanohashidate - Kyoto (?) ¥ 4,380 of which ¥ 3,000 is covered
This adds up to ¥ 30,020 the 7 day JR Pass is ¥ 28,300 giving you ¥ 1,720 in savings. This is assuming that you'd be back in Kyoto on the last day that the 7 day JR Pass is valid. Remember that the JR Pass can also be used for local travel within Osaka and Kansai, possibly adding more to your savings.
The JR Pass can also be used on local JR Buses but I am not sure if this would be of use with the your itinerary.
