The JR Pass covers travel between all of the above places. You can find the train schedule using Hyperdia.com.
Online purchase for Disney ticket is recommended, this is a bit cheaper and saves you queuing.

You can buy day passes at the Tokyo Metro ticket machines, these are about 750yen per day.
Where could we buy the local sim card for our mobile phone?
